Patients should obtain a referral if needed prior to the visit.
Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule. Everyone at our office is at your service.

Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice.  This courtesy on your part will make it possible to give your appointment to another patient.

Emergencies
If you have a medical emergency, our office provides an on-call service, 24 hours-a-day, 7 days-a-week.

Managing Health Care Costs Through Our In-House Billing Services
Seidenberg Protzko Eye Associates is committed to keeping your eye care costs at a minimum while providing you with the highest quality experienced eye care:

• We electronically file to various insurance companies including Medicare and Blue Cross/Blue Shield. We also file HMO & PPO claims for those payors with whom we participate. Please call our office for more information; also, please remember that your insurance company may require you to bring a referral for your visit.
  We welcome and will assist you with any insurance questions you may have.
• We accept assignment with Medicare and Blue Cross/ Blue Shield as payment in full providing that your deductible/co-payment has been met.
• We gladly accommodate Worker's Compensation cases by verifying work-related injuries, providing emergency treatment and claim filing.
• Payment for other services and co-pays are due at the time of your visit, so please supply our office with the most current information about your coverage. If you need financial assistance, please let us know so we may help you.
