Insurance & Financial Policy


We are contracted with most major insurance carriers. Your health insurance policy is an agreement between you and your insurance company. Payment of benefits is subject to a member’s eligibility on the date service was rendered and to any other contractual provisions of your employer’s plan.  Your insurance carrier makes the decisions as to what is covered and what will not be covered. Please be prepared to provide our front desk with the following information so that we may submit your claims accurately.

Remember to Bring Your:

  • Current Insurance Cards
  • Driver’s License
  • Primary Care Referral (if required by your insurance)


Seidenberg Protzko Eye Associates is considered a specialty practice with some insurance carriers. Therefore, if your insurance requires a primary care referral to see a specialist, you are required to contact your primary care provider prior to your appointment and present your referral at the time of your appointment.

Fees and Payments

Our philosophy at Seidenberg Protzko Eye Associates is to provide the highest quality eye care at a reasonable price. To assist us in meeting this goal, please be prepared to pay all office co-pays and non-covered services at the time of your appointment. Self-pay patients are required to pay in full at the time of service. If financial assistance is required, please ask to speak with our billing department.

Workers Compensation

Please advise our front desk if your condition is related to an injury on the job. Our office will notify your employer for verification and assist you in paperwork.

Third Party Liability

When making your appointment, please advise us if your condition is related to an auto accident. We can bill your auto insurance when you supply your auto insurance information to us.
